Cap: Tan/light brown with a slightly darker center; margins were lined with bumps; large veil fragment on cap
Gills: White; free; some short gills
Stipe: No ring; white; equal; fibrillose; ending in a membranous volva
Habitat: Growing on the ground under oak in a mostly deciduous forest

A universal veil completely encloses the developing mushroom. It either disappears as the mushroom develops, or it leaves behind warts or patches on the cap and/or bits (called volva) at the base of the stem.
A partial veil covers the gills/pores of maturing mushrooms (to protect them until they are mature and ready to release spores). It either disappears or forms a ring/ring zone on the stem as the mushroom matures.
The mushroom in this photo has an example of universal veil remnants on the cap.
