Cap: Tan/light brown with a slightly darker center; margins were lined with bumps; large veil fragment on cap
Gills: White; free; some short gills
Stipe: No ring; white; equal; fibrillose; ending in a membranous volva
Habitat: Growing on the ground under oak in a mostly deciduous forest
